To apply for a 1st Provisional Licence you will need :
- Theory Certificate
- Completed Application Form
- A Birth Certificate
- Marriage Certificate (if appropriate)
- An eyesight report for classes A, A1, B, EB and W - eyesight report valid for one month from issue
- A medical report (valid for one month from issue) for classes C, C1, D, D1, EC, EC1, ED and ED1
- Two Photographs (signed on the back)
- Fee for 2 years - €15.00
To apply for a 2nd Provisional Licence you will need :
- Completed Application Form
- Two Photographs (signed on the back)
- Previous Licence (if previous licence is unavailable, a lost licence declaration stamped at the Garda Station is required.
- Fee of €15.00
To apply for a 3rd or Subsequent Provisional Licence you will need :
- Completed Application Form
- Two Photographs (signed on the back)
- Failure Cert (valid for two years only)
- Previous Licence
- Appropriate Fee
If you have not sat a Driving Test in the last two years you must submit :
Receipt from Driving Test Section, The Road Safety Authority, Moy Valley Business Park, Primrose Hill, Ballina, Co. Mayo., (Locall: (1890) 40 60 40 Fax: (096) 78290 Email: drivingtest@rsa.ie Web:www.drivingtest.ie) with actual date of upcoming appointment. In this case a 1 year licence will only be granted.
