FORM RF100A - MOTOR TAX RENEWAL
This form can be used to renew the motor tax on your vehicle if, for any reason, the form issued to you in the post is unavailable. The form must be used if you wish to pay arrears in a motor tax office. Detailed instructions on completing and submitting the form are contained in the accompanying notes.
FORM RF105 - TRANSFER OF VEHICLE OWNERSHIP TO A MOTOR DEALER
This form must be completed when notifying the Department of a transfer of vehicle ownership to a Motor Dealer. The form must be completed by both parties and forwarded to the Department of Transport, Tourism and Sport, Town Centre, Shannon, County Clare. The seller should also give the motor dealer the Vehicle Registration Certificate (or licensing certificate).
FORM RF111 - CHANGE OF PARTICULARS
This form is to be used if a change is required to the recorded details of your vehicle, such as:
- Motor tax class
- Engine details
- Name (as a result of marriage, etc.)

FORM RF111A - GOODS ONLY DECLARATION
This form must be completed at a Garda Station if the applicant is taxing the vehicle at the goods rate and the design gross vehicle weight of the vehicle does not exceed 3,500 kg.
FORM RF200 - NOTIFICATION OF TRANSFER OF OWNERSHIP
This form is to be used if you require to notify change of ownership of a vehicle registered before 1 January 1993. The completed form and logbook must be submitted to your local motor tax office. Changes of ownership of vehicles registered since 1 January 1993 should be notified by completing Part B of your Vehicle Licensing Certificate and returning it to the Department of Transport, Tourism and Sport, Town Centre, Shannon, County Clare.
FORM RF134 - APPLICATION FOR REPLACEMENT DOCUMENTS
This form is to be used when making an application for a replacement document where the original was lost, stolen or destroyed. The form must be completed and witnessed by a member of the Garda Síochána at your local Garda Station. The completed form must be submitted to your local Motor Tax Office.
FORM RF120 - APPLICATION FOR A REFUND OF MOTOR TAX
This form is to be used when making an application for a refund of motor tax. You must complete the form and get it witnessed by a member of an Garda Síochána. The completed form must be submitted to your local Motor Tax Office.
FORM RF150 - DECLARATION OF NON-USE OF A MOTOR VEHICLE
This form is to be used if declaring a period of non-use for a vehicle. The completed form must be submitted to your local Motor Tax Office.
